Implant Reaction Survey

Thank you for taking this brief but very important survey. Our goal is to query as many patients as possible who have had implants to help understand whether they reacted to their implant and qualify their symptoms. If you have had your implant removed, we would also like to hear from you.
 
This survey is an international collaboration between patient advocates and doctors hoping to learn more and thus educate others in order to advance medical care. This is a completely anonymous survey.
