Self Advocate Survey

Introduction

Thank you for taking our survey. This survey is for individuals who are served by a regional center. We are asking questions to get a better idea of how the direct support professional (DSP) workforce shortage affects people with disabilities.

The survey will take about 5 minutes to answer. The survey will remain open until June 30, 2024.
The answers we collect in this survey will not have your name on it. Your personal information will be kept private. Results from this survey will be published in a report, presented at conferences, and shared with people who are interested in learning more about the DSP workforce shortage.

Your participation is voluntary. You can choose to not participate, or you can end your participation at any time.
