Epilogue Feedback Survey

Epilogue is the official bereavement newsletter of Faith and Hope Hospice.
The newsletter is one of the ways we support our clients currently receiving bereavement support and serves as a resource for the community too.

Epilogue provides information on grief and the grief process by providing:
-       Ways to understand & cope with grief
-       Resources
-       Cultural touchstones
-       Artistic expressions of grief

We strive to continually improve our bereavement services, including the newsletter, and want to ensure that the services we provide match the needs of our clients. So, we want to hear from you. We would like to know your thoughts on the newsletters you received as part of the yearlong bereavement support. Let us know if you found them helpful and if there is other information that we should include or remove.

This short, 10-item survey should take 5 minutes or less to complete. We would greatly appreciate your feedback and participation.

If you have any questions or desire additional information, please don’t hesitate to contact our Bereavement Services Coordinator, Reid Jacobs, at 818-559-1460 or email reid@faithandhopehospice.com
