We believe your experience and knowledge of logistics and transport can certainly help us to solve the problems in the industry. Please answer all questions. 

The survey consists of questions related to your experience and knowledge of  supply chain and logistics risks, and logistics performance in your organisation. There is no right or wrong answers. Your responses will be strictly confidential. The data collected through the survey will be analysed and the results may appear in publications. There are four different categories of supply chain risks include logistics risks, information-related risks, customer-related risks, and environment risks. It seeks to find out how you consider the severity of each risk variable in your company recently.
