Parent Survey: Covid-19 vaccine interest for 12-18 year old students

Grayson County Health Departments asks that ONE adult or caregiver per household complete the following survey. We appreciate your participation as it helps us to prepare for how much vaccine is needed when the Pfizer Covid-19 vaccine is approved for children 12 and up.
