Survey Introduction
PURPOSE:
The survey is part of a 5W improvement project aimed at helping us coordinate care, especially for patients with medical needs and behavioral/psychological comorbidities.
Information from this survey will be used to identify opportunities for improvement.
IMPORTANT DEFINITIONS:
-All questions refer to 5WA patients with medical AND behavioral/psychological comorbidities only.
-For example, patients with malnutrition and an eating disorder who are also aggressive at times, or a patient with cellutilis who is non-verbal with autism.
-All other patient types on 5W are out of scope when answering the survey.
SURVEY INSTRUCTIONS:
-Rate your level of agreement for each statement using the 1-5 scale provided (e.g. 1 = Strongly Disagree, 5 = Strongly Agree).
-Select "Not applicable" for statements whose content does not apply to your role (e.g. statement pertains to patient rounds and your role has no connection to that process).
