Instructions
This is a screening survey for people interested in the "Mother Up" program. Mother Up is a new guaranteed income research program, created by the nonprofit Mother's Outreach Network. If you end up entering the Mother Up program, you will also be entering a research study.
To be eligible for the program, you must:

- Be age 18 or older
- Be Black/African-American
- Live in Washington, DC
- Earn an income at or below 250% of the federal poverty line (those who are eligible to receive public benefits meet this requirement)
- Be a biological mother to a child age 14 or younger who currently lives in your household and for whom you have custodial responsibilities
- Have had current or recent involvement with the DC Child & Family Services Agency within the last year, for your biological child age 14 or younger
- Not be receiving SSI benefits*
*Participation in the Mother Up program could result in the loss of some or all of your SSI benefits.
Everyone in the program will get access to Mother's Outreach Network's legal program and will have the opportunity to receive incentive payments for participating in research like surveys and interviews. Half of the people in the program will also receive $500 a month, no work and no other strings attached. A computer will randomly select which participants will be given the $500 a month.
By taking this screening survey, you are saying you're interested in the program and that you want us to follow up with you to give you more information. If you are eligible for the program, we will follow up within the next couple weeks to give you an application. Not all eligible applicants are guaranteed to be selected for the program.
