About our survey

If you need any help completing this survey please contact Scott Bacon on 07421 071820.

Youth Out Loud are creating a podcast where they will discuss and raise awareness of the barriers for young people with disabilities in Kingston accessing local NHS and social care services.

Barriers to accessing service are anything that is stopping you from using that service. They could be the timing of the service, the location of the service, lack of transport, lack of information, an over stimulating environment when you arrive, whatever is making it difficult for you to access the service.

Your voice could inform future provisions to help improve access to services and we also want to hear what would make things better for you.

You can complete our short survey for yourself or for someone you know with a disability.
