As part of SPARC Group’s compliance program, all vendors of finished goods (Tier 1) and materials/components (Tier 2) for any SPARC Group brand (Aeropostale, Lucky Brand, Nautica, Brooks Brothers, Forever 21, Eddie Bauer, or Reebok) are required to complete this Supplier Sustainability Survey for each facility used by vendor for SPARC Group brand products.
 
The information provided will only be used internally at SPARC Group to begin understanding environmental sustainability practices within our supply chain.  This information will NOT be shared externally or with other suppliers.  Please answer truthfully and to the best of your ability.
 
All vendors must complete this survey for all facilities that produce finished goods, materials, or components for one or more SPARC Group brands or forward this email to the appropriate contact at those facilities. 
  
Example 1:
  • ABC, Inc. has 3 separate facilities (i.e., buildings with unique addresses) of which only 2 produce apparel garments for SPARC Group brands.
    • Facility 1 produces for Aeropostale and Nautica --> Complete survey for this facility
    • Facility 2 produces for Aeropostale --> Complete survey for this facility
    • Facility 3 does not produce for any SPARC Group brands --> Do not complete survey for this facility
Example 2:
  • XYZ, Inc. has 2 separate facilities (i.e., buildings with unique addresses) of which both produce apparel garments for SPARC Group brands.  XYZ, Inc. also contracts with a separate vendor (ACME, Inc.) with 1 facility that produces fabric used in the apparel garments that XYZ, Inc. produces for SPARC Group brands. 
    • XYZ, Inc.’s Facility 1 produces for Eddie Bauer --> XYZ, Inc. completes survey for this facility
    • XYZ, Inc.’s Facility 2 produces for Forever21 --> XYZ, Inc. completes survey for this facility
    • XYZ, Inc. must send survey link and instructions to ACME, Inc.
      • ACME, Inc.’s Facility A produces fabric for XYZ, Inc.’s Facility 1 and Facility 2 --> ACME, Inc. completes survey for its Facility A

This survey cannot be saved and therefore must be completed in one try, please make sure you have all information need to complete this survey prior to taking it and review the tutorial prior to taking the survey.

SPARC Group views this survey as mandatory.  SPARC Group reserves the right to verify the information provided including via onsite facility visits by SPARC Group personnel or third-party verifiers.
