Call for Nominations

CPhA is seeking nominations for the Canadian Pharmacist of the Year Award to be selected from the community of vibrant and diverse pharmacists across the country.

Nominations may be submitted by:
  • Any pharmacist who is a member of one of CPhA's Organizational Members (AFPC; AQPP; BCPhA; NBPA; OPA; PANL; PANS; PAS; PEIPhA; Pharmacists Manitoba; RxA)
  • CPhA Organizational Members
The nominee could be a recent winner of another award in your province or organization or a pharmacist who deserves to be recognized for the first time. In addition to the list of criteria below, we encourage nominators to put forward a nominee who uses innovation, collaboration and compassion in their practice and demonstrates a commitment to tackling systemic inequities and addressing gaps in our health care system through their work.

All nominees must be current members of one of CPhA's Organizational Members. The award winner will be selected from among all nominations CPhA receives, both from Organizational Members and from individual pharmacist submissions. Selection of award recipients is made by the CPhA Awards Committee, subject to confirmation by the CPhA Board of Directors.

To submit your nomination online, you will need the following:
  • 3 nominators (all nominators must be current pharmacist members of one of CPhA's Organizational Members)
    • Membership #s are required for all 3 nominators and the nominee
  • A nomination summary describing how the nominee meets the award criteria (maximum 4 pages)
  • The nominee's CV or biography (maximum 5 pages)
  • A signed consent form by the nominee accepting the nomination for the Canadian Pharmacist of the Year Award and giving consent to CPhA to perform a background check - please download this consent form to be signed by the nominee and submitted with your nomination
Optional: You may choose to provide up to 3 supporting documents (e.g., testimonials, presentations, articles, etc.) limited to the past 5 years.

Disclaimer: As part of CPhA's commitment to upholding the highest standards of integrity and excellence, a background check will be conducted on all award nominees. Any information uncovered will be kept confidential and will be used solely for the purpose of evaluating the nominee's suitability for the Canadian Pharmacist of the Year Award. A background check will not be undertaken without the consent of the nominee. A background check may include a review of any professional disciplinary action, criminal convictions, social media behaviour, and/or other legal or ethical matters that are inconsistent with the values and principles of the award.
