A Wellness & Self-Advocacy Project

Thank you for your interest in participating in We Are ILL’s Empowering Black Women with MS program, designed to provide education, mental health services, and advocacy training for a select group of this aforementioned community based in the Atlanta metropolitan area. This intake form will help us understand your needs and ensure that we can provide you with the best possible support.

This program will take place in four installments (July 11, 2024 - in person, and tentatively August 15, 2024; October 17, 2024; and February 27, 2025 - virtual), in collaboration with Shepherd Center in Atlanta, GA.

We Are ILL is requesting data from you to better inform the organization about the applicants. Please take a few moments to complete the following questions.
