* 1. Restaurant Customer Preferences Informed Consent Form

I am conducting a survey on consumer opinions. Your voluntary participation is requested so I may learn more about consumer opinions. This survey poses no foreseeable risk to you as a participant. This survey will take approximately 20 minutes. Your name will not be recorded on the questionnaire and your responses will be anonymous. Again, your participation is voluntary and you may choose to not answer all of the questions on the questionnaire even after signing the consent. If you are willing to participate, please sign this form.
