Consent and acknowledgment (Please read carefully.)
My name is Carla Browning. I am a graduate student seeking my Master of Public Administration degree at the University of Alaska Southeast. I am conducting a survey to assess organizational development needs at various levels of the University of Alaska as part of my final Capstone course, PADM 690-JD1. My research project has been reviewed by the Institutional Review Board. (IRB #1975385 approved Nov. 18, 2022). If you'd like to participate in this research, please read the following consent statement and indicate your willingness to continue below.
Statement of consent:
I am an employee of the University of Alaska. I am 18 years of age or older. I understand the information presented to me. My questions have been answered, and I agree to participate in this survey.

Study details:
· The information may be used to make organizational improvement recommendations in my Capstone paper, published in aggregate, and compared with future data.
· The information could be used to find solutions to common problems and improve organizational performance, training programs and job aids.
· This study provides you with an opportunity to share your experiences. For some, that may be uncomfortable.
· Although IP and email addresses will not be collected, there is minimal risk that you could be identified.
· There are opportunities to provide open-ended feedback. Please be respectful and provide solutions where possible.
· Any personally identifiable data will be kept confidential and securely maintained by the researcher.
