This 5–8 minute survey asks about services, healthcare, community participation, accessibility, quality of life, and well-being. Responses are confidential. The Autism Society Inland Empire is a safe space. We are not mandated reporters, and we encourage you to share your experiences and needs honestly.
We may use survey information for planning, programs, grants, advocacy, reports, and presentations. Anonymous quotes may be shared. Please do not include names or identifying information in written responses.
