USTA Nebraska Hall of Fame Nomination Form

Anyone may make a nomination to the USTA Nebraska/NTA Hall of Fame. Nominees may be living or deceased. These are the criteria:

• Player or contributor while living in the Nebraska District for at least 5 years

• Sportsmanship and character of a high standard

• Contribution as solely a player must be of national caliber. Those who are not players of that caliber must have been involved in multiple areas for at least 15 years, such as the following:

* Player
* Coach
* Administrator
* Sponsor/promoter
* USTA leadership
* Tournament/league official
* Teacher
* Manufacturer

• Contributions primarily in a paid position must have been supplemented by volunteer contributions in other areas.
