HOW ARE THE WINNERS SELECTED?
The selection committee includes Outpatient Surgery Magazine editors and AORN ASC subject matter experts. Submissions in each category will be rated based on the details you provide about how the use of teamwork, innovation, collaboration and/or leadership facilitated problem identification, project planning, implementation, monitoring and measures of success. Winners will be featured in the September issue of Outpatient Surgery Magazine, announced at the OR Excellence Conference and honored at a ceremony/event to be announced later.
HOW DO I SUBMIT A NOMINATION FORM?
To enter, simply indicate the category (or categories) in which your facility has achieved sustained success in improvement projects, initiatives or practice changes by filling in the text box below each category. Multiple category submissions are accepted. The submission deadline is Friday, July 26, 2024.
WHAT ARE JUDGES LOOKING FOR IN THE SUBMISSIONS?

Problem Statement: Briefly state your known or suspected problem or opportunity for improvement.
Describe the situation or process and clearly articulate the problem or opportunity. Explain how this problem or opportunity affects the facility.

Goal Statement: Describe the level of performance or goal you aim to achieve.
Define the specific, measurable, achievable, relevant and time-bound goal(s).
Data Collection: Provide the data or evidence supporting the existence of the problem.
Define the results of the data collected and explain the trends, patterns or root causes identified.
Action Plan: Describe the action plan created to meet the goal(s).
The steps of the action plan, including roles, responsibilities, milestones and timelines.
Evaluation: Describe the process to evaluate the results of the plan.
Describe the data collected and compare the results with the baseline data and goals.
Measure of success: Describe the process to measure the outcomes over time.
The process and schedule used to collect data over time and what measures were sustained when you stopped collecting it, as well as any additional information you feel sets your project apart and why it is worthy of the OR Excellence Award.
