Notice: Completion of this form is required to satisfy annual reporting requirements under Federal Incidental Take Permit No. TE10064. Personally identifiable information collected will be used for program administration and may be provided to requesters as required by Wisconsin's Open Records law [ss. 19.31-19.39, Wis. Stats.]. 
 
The annual report ensures that HCP partners are minimizing impacts to Karner blue butterflies during the course of their routine work and are supporting the continued existence of Karners on their land.

All partners of the Karner Blue Butterfly Habitat Conservation Plan are required to complete and submit to the department an annual report of activities conducted under the Federal Fish and Wildlife Permit TE010064-6 by March 1st.
 
Failure to submit timely, complete and/or accurate annual reports jeopardizes the continuation of the HCP program in Wisconsin. Without the HCP program, each partner would need to apply to the USFWS for an individual incidental take permit, which is a time consuming process. We appreciate your attention to meet the March 1st deadline and effort to maintain the HCP program.

Prior to starting your annual report, please have the following documents available for reference:
  • Information on changes in staff who work on the HCP
  • Completed Lupine and Karner survey forms
  • Records of outreach and education activities
  • Records of land management activities conducted on KBB occupied sites

The DNR and HCP partners are required under legal binding agreements to provide the USFWS with the specific information collected in this annual report. The annual reports verify that HCP partners are meeting the conditions of the HCP.
