Differences in practitioners’ approach towards orthopedic surgery based on location.

.State the location (city, state, and zipcode) of your facility or place of veterinary work. If mobile unit give general area.(Required.)
.Please indicate your style of practice.
.The veterinarians at my clinic/hospital perform orthopedic procedures (such as Tibial Plateau Leveling Osteotomy (TPLO), Femoral Head Osteotomy (FHO), Arthrodesis, Luxating Patella Repair.
.My clinic/hospital offers orthopedic procedures with the help of a mobile or traveling veterinarian that comes into our facility and performs orthopedic specialty procedures.
.At our facility, referrals are the first option for treatment of orthopedic procedures (including cranial cruciate ligament repairs), but veterinarians at our facility will perform orthopedic services when referrals are not feasible based on logistics or cost.
.For patients of your clinic/hospital needing orthopedic procedures, referral to another veterinary clinic/hospital is the only option.
.If your clinic/hospital sends patients for referrals for orthopedic procedures, do you refer to private referral hospitals, traveling veterinary services, or universities? If comfortable, please list the name of your referral resources/centers.
.If your clinic/hospital offers self-performed orthopedic services without the assistance of a mobile or traveling veterinarian, please select all procedures offered at your clinic/hospital. If none, please enter N/A in the “Other” textbox.
.If your clinic/hospital uses a traveling orthopedic veterinary service or you are a mobile or traveling orthopedic veterinary service, please select all procedures that you offer. If none, please enter N/A in the “Other” textbox.
.If you perform orthopedic services, please indicate if you are board-certified or if you had additional training unrelated to board-certification.
.If you perform orthopedic services, please explain how you obtained the training for the procedures you perform. (For example, did you attend continuing education on a specific procedure?)
.If you or your fellow practitioners at your facility do not perform orthopedic procedures (including cranial cruciate ligament repairs), what are the reasons these services aren’t offered?
