Thank you for participating in the annual survey of the Department of Youth Faith Formation in the Archdiocese of New York. Your feedback is essential for understanding trends and planning for the future.

Ideally, one person from your parish should complete the survey for all the grades (early childhood to high school youth ministry). However, if multiple people are completing the survey (e.g. the DRE and Youth Minister), put 'zero' in for the areas you are not responsible for. Most questions require an answer.

If you are a merged parish with more than one location for faith formation, please add the data from all locations and enter into the survey.

If you oversee parish faith formation in multiple churches that are not merged, please enter in information for each parish separately.
This survey will take approximately 10 minutes to fill out, providing you have the information needed. It is best to prepare by downloading a copy of the survey, completing it on paper and then come back here to input.

IMPORTANT NOTE: If you are part of a recently merged parish but both parishes are listed, choose the name of one of the parishes to do your report under. In the section for YOUR name, please also include the name of the other parish(es) that were part of the merger.
