2024-2025 School of Architecture Faculty/Staff Operational Request Form

This form should be utilized to make any operational request. This can include but is not limited to purchase requests, student hire requests, scheduling requests and visitor parking requests. Please utilize this request form throughout the semester as your needs arrive. Feel free submit multiple requests in a submission. Please make sure to plan ahead for all requests as there can be delays in shipping time and other approvals.

I would like to make a specific office supply purchase request. Please provide as much information as possible regarding the item/s. Include vendor and link if possible.
I would like to make a specific non-office supply purchase request. Please provide as much information as possible regarding the item/s. Include vendor and link if possible. Additionally provide detailed rationale/reason for the request. Please included information on how the purchase can be justified as a departmental expenses (example - specific resource to enhance a particular course)
I would like to request a MSU Garage Parking Code for a course guest. Please list specifics including the date of the visit, name of the visitor and the class that visit is associated with. Please note that parking codes will be generated for Final Review Critics automatically based on the review list. Those requests do not need to be made individually.
I would like to request a Room Reservation for a non-School of Architecture controlled space (JLF Conference Room, NAH, REID, JABS, etc). Please include as much information about the event as possible including date, time, event information, number of attendees and purpose of the event. Please note that if you are wanting to schedule Architecture controlled spaces you should use the appropriate sign-up calendars to schedule (CHVR 110, 131, 148, Lower Gallery, Upper Gallery and Crit Spaces).
I would like to make a request that does not align with any of the other choices listed above. Please provide as much detail as possible regarding your request.
