This survey, The Hazard-Threat Assessment Survey (HTAS) is designed to subjectively prioritize possible threats or hazards your organization may face. As you work through the questionnaire read it as your own perception of possible hazards or threats and at your present level of understanding for what each risk might mean. If you are not familiar with a particular threat, just skip it and move on to the next one. This only requires your opinion and it need not reflect what others might think.

The survey consists of four categories to be rated. They are:
  • Probability (how likely you think it is this will occur)
  • Magnitude (how bad you think it is going to be)
  • Warning (how far in advance might you know it is going to occur)
  • Duration (how long do you think the event will last – when ongoing damage or threat stops)
Select the answer that first comes to mind and don’t overthink your answers.
