Thank you for attending AOCA's 2022 VAPOR Annual Convention. This is an online ATTESTATION FORM for you to record your attendance and evaluate the conference.
PLEASE READ THESE DIRECTIONS CAREFULLY:
1. ATTEST: Please attest to your participation to claim Category 1-A AOA Credit. This form is mandatory to complete if you wish to have your credit reported to the AOA on your behalf.
2. REPORTING TO AOA: Your information will be reported to the AOA on your behalf by the completion of this form.
3. PRINT YOUR CERTIFICATE: Once you have completed the CME statement of attendance, you will be directed to a page where you can fill out and print your own CME certificate. Please note your total number of hours to fill out your certificate accurately.
