Participant Information Sheet

You are invited to participate in this research project. Prior to deciding whether you want to participate or not, in this research, it is important for you to understand why I am conducting this project and what it will involve. Please read the following information carefully.
What is the purpose of the research?
I am researching Swansea University students’ perception on vaccines and community pharmacies, in Wales, involvement in vaccinations. The study aims to understand Swansea University students’ perception on vaccines and pharmacies involvement in vaccinations by asking students to complete a questionnaire. By carrying out this questionnaire I hope to analyse the data and come to a conclusion regarding students’ perception on vaccines and pharmacies involvement in vaccinations.

The purpose of the study is to discover and understand students’ opinion and see if it matches with current data regarding vaccine uptake, specifically the MMR vaccine and to evaluate and suggest ways that pharmacies can be better utilised in regard to vaccinations, such as the MMR vaccine.

To participate in this study, you will need to complete a questionnaire. This whole process including the questionnaire should take less than 10 minutes of your time.


Who is carrying out the research?
The data that is being collected is going to be used by myself in my project, which may be published. This study has been given a favourable ethical opinion by SUMS RESC (Project Reference Number: 9046).


What happens if I agree to take part?
There will be a short questionnaire, that I kindly request that you rate your response to the statements in the survey using a scale that ranges from strongly disagree to strongly agree. Remember to answer honestly.


Are there any risks associated with taking part?
This study has been given a favourable ethical opinion by SUMS RESC (Project Reference Number: 9046). There are no significant risks associated with participation.


Data Protection and Confidentiality
Your data will be processed in accordance with the Data Protection Act 2018 and the UK General Data Protection Regulation (UK GDPR). All information collected about you will be kept strictly confidential. Your data will only be viewed by the researcher.

All electronic data will be stored on a password-protected file on a university computer located within the secure and pass protected building at Swansea University. Only the research team, which includes myself, 2108269@swansea.ac.uk and my project supervisor Dr Juman Al-Dujaili, juman.dujaili@swansea.ac.uk have access to this password protected file. This study has been given a favourable ethical opinion by SUMS RESC (Project Reference Number: 9046). All paper records will be stored in a locked filing cabinet at Swansea University. Your consent information will be kept separately from your responses to minimise risk in the event of a data breach. By clicking the ‘Next’ button on the survey, you are confirming that you are giving your informed consent to participate in this study.


GDPR Compliance note:
Please note that the data we will collect for our study will be made anonymous, any notes taken, or transcripts will not be possible to identify and remove your data later, should you decide to withdraw from the study. Please note that as this information is being collected online, once the data has been submitted online you will be unable to withdraw your information.
What will happen to the information I provide?
An analysis of the information will form part of our report at the end of the study and may be presented to interested parties and published in scientific journals and related media. Please note all information presented in any reports or publications will be anonymous and unidentifiable.


Is participation voluntary and what if I wish to later withdraw?
Your participation is entirely voluntary – you do not have to participate if you do not want to. However, if you want to withdraw from the study, before submitting your questionnaire, then you are free to withdraw, without giving a reason. Please note once you submit your online questionnaire, we are unable to withdraw your information, as there is no identifiable information gathered to withdraw your specific data from the study.


Data Protection Privacy Notice (Required statement for GDPR compliance)
The data controller for this project will be Swansea University. The University Data Protection Officer provides oversight of university activities involving the processing of personal data and can be contacted at the Vice Chancellors Office.

Your personal data will be processed for the purposes outlined in this information sheet. Standard ethical procedures will involve you providing your consent to participate in this study to provide consent you will have to tick the boxes. These boxes show that you have read the participant information sheet and consent form, and are confirming your informed consent to take part in this study and will complete the questionnaire that has been provided to you.

The legal basis that we will rely on to process your personal data will be processing is necessary for the performance of a task carried out in public interest. The public interest justification is approved by the Medical School Research Ethics Committee, Swansea University.

The legal basis that we will rely on to process special categories of data will be processing is necessary for archiving purposes in the public interest, scientific or historical research purposes or statistical purposes.



How long will your information be held?
Data will be preserved and accessible for a minimum of 10 years after completion of the research. Records from studies with major health, clinical, social, environmental or heritage importance, novel intervention, or studies which are on-going or controversial should be retained for at least 20 years after completion of the study. It may be appropriate to keep such study data permanently within the university, a national collection, or as required by the funder’s data policy.


What are your rights?
You have a right to access your personal information, to object to the processing of your personal information, to rectify, to erase, to restrict and to port your personal information. Please visit the University Data Protection webpages for further information in relation to your rights.
