- live or work in Lowell, Dracut, Chelmsford, Tyngsborough, Tewksbury, Billerica, Westford, or Dunstable;
- and consent to participate

This survey will ask you about:

- basic information about yourself (like your age and town you live in)
- your thoughts on community health and resources
- your experience with health care and resources

Your answers are confidential and anonymous; you may skip questions at any time.

In recognition of your time, at the end of the survey you can choose to provide contact information to be entered to win one of several gift cards. You will be redirected to a different site to enter your information, if you decided to participate; your contact information will NOT be connected to your survey answers.
