To receive Proof of Attendance Certificate:
Attendees can request a Proof of Attendance certificate by submitting the Proof of Attendance form information below.
All fields in the form are required. Please do not submit incomplete forms. All incomplete submissions will be deleted. A certificate will be emailed to the email address provided in the request form. Each attendee wishing to receive credit should complete the request form one (1) time
Attendees wishing to get an attendance certificate should complete this form by June 1, 2024. Please note these forms will not be reviewed until the week of June 3, 2024.
If you submit this form following the June 1st deadline, please reach out to intern@nyla.org to confirm your submission and receive your certificate. This form will not be reviewed past the deadline noted unless requested.
