1. Welcome to USICG's Annual Diocesan Retirement Survey
To better serve you and to provide a more comprehensive summary, we are providing this questionnaire to all Catholic Dioceses and Archdioceses. Please complete the survey with current information on your retirement plans. It only takes an average of 5-10 minutes to complete the survey! All information is confidential.
This survey can be used for assessing how your retirement benefit offerings compare to other Dioceses and Archdioceses across the country. We are happy to include additional fields in the future that you may find helpful.

* Each survey respondent will be entered into a drawing to win an Amazon gift card. Three (3) winners will be randomly selected and contacted via the email address provided. Winners will receive a gift card in the amount of: 1st name drawn - $50; 2nd name drawn - $50; 3rd name drawn - $25.
