1. Introduction

The Community Council would like to collect additional up-to-date data related to Social Determinants of Health (SDOH). SDOH makes up the conditions in the environments in which people are born, live, learn, work, play, worship, and age that affect a wide range of health, functioning, and quality-of-life outcomes and risks.

This screening tools ask about various topics such as family and community support, education, physical activity, substance use, mental health, and disabilities. The purpose of these questions is to establish priority needs based on social issues that deter an individual's and a community's overall health. Please note that this survey is anonymous.
The Woodfords Community has received funding through the American Rescue Plan Act to help communities recover from the effects of the pandemic and to meet the needs of the people and create a safe and healthy community.
