  • This form is used for recording all vaccinations and other herd health products given to the cow herd and bulls annually. It is a required form that must be completed before calves can be marketed as certified by the Integrity Beef Alliance.
  • As a minimum, only program approved products are allowed.  You may use additional products, but at a minimum you must use the products specifically outlined in the program health protocol which may be found on www.integritybeef.org/forms.
  • If you have not worked the cows for this year, before the calves are marketed, then use the information from the last time the cows were vaccinated. 
  • At the end of the survey click the "DONE" button to submit the answers. 
  • You will receive an email after you click the "DONE" button with a link to the questions and your answers. Be sure to print the responses out as the link is only active for 90 days. Retain this information for your records please.
