2021 Summer Program Survey

It's time to start planning for summer, and we want to make sure that we have up-to-date information about your summer program available to families and community partners.

This survey should take approximately 10 minutes to complete. The information you provide about your program will be made available to the public for the purpose of helping schools/districts, program providers, community partners, families, and other stakeholders plan for and make decisions about summer learning this year.
