The Central New Hampshire Health Partnership wants to learn about the health of your community and we want to hear from YOU.

“Your community” can mean different things to different people. For the purpose of this survey, your community can be as big as all of Central New Hampshire or as small as your neighborhood. Answer the survey questions by thinking about the area you see as “your community.”
