Answer the following questions for your 2018 high school & middle school softball registration. Registration must be completed by January 15 of each year. A late fee of $20.00 will be charged for fees paid after January 15. There is a return check fee of $25.00.

* 6. I agree to serve as an independent contractor with respect to any assignment that I accept. Furthermore, I agree to hold the Triangle Softball Officials Association, Triangle Sports Officials, LLC and the Regional Supervisor harmless and free from any and all liability for injury or damage sustained as a result of any assignment. I hereby certify that I am in good physical and mental condition to officiate the games to which I am assigned. I will also inform the regional supervisor of any schools that I have a conflict of interest (child attending, family or friend of coach or staff member, recent graduate). If I am then assigned to that school I will notify the supervisor so my assignment so it can changed.
    I understand and agree that this registration is for one year only. Further, I understand that registration does not carry any obligation on the part of the Triangle Softball Officials Association, Triangle Sports Officials, LLC or the Regional Supervisor for a specific number or type of assignments. Any assignment, which I receive, is subject to cancellation by the Regional Supervisor if he deems such cancellation to be in the best interest of the official or the Triangle Softball Officials Association or Triangle Sports Officials, LLC. I also agree to follow all policies of the NCHSAA and Triangle Softball Officials Association.  Applying for membership in the Association is voluntary and entails the understanding that the Association is a private organization that places restrictions and requirements on its members. Restrictions or requirements which may be mandated include various standards of dress, grooming, and behavior while engaging in any activity related to the Association. One such restriction is the prohibition on any public display of political or social protest while on any assignment from the Association. By applying for membership, the undersigned agrees to be bound by any standard or restriction imposed by the Association or Regional Supervisor of Officials.
   By typing my name below and paying my registration fee, I agree that I am electronically signing this registration, agreeing to the terms of membership of the association and that I have not been convicted of a misdemeanor involving moral turpitude or a felony. I further agree that if I am charged with a misdemeanor involving moral turpitude or a felony, I will notify the booking agent immediately.
   I understand that this registration is not complete until my registration fee has been paid.
