2015 USA Ultimate U-16 Youth Club Championships Application (Phase 2)

Thank you for your interest in attending the U-16 division 2015 USA Ultimate Youth Club Championships. This new online form will save time and allow us to process applications in a more efficient manner.

The 2015 USA Ultimate Youth Club Championships will be held in Blaine, MN on August 7-9.

Phase 1 bids have been awarded in the open division. However, there is still the possibility of earning a bid in Phase 2.

Phase 2 applications must be completed by 5pm MT Monday, May 25th. Bids will be awarded according to the team selection process on June 5, 2015.
- USA Ultimate-sanctioned Youth Club Leagues with multiple age divisions must fill out the application by the deadline.
- At-large applicants must fill out the application in order to receive instructions for the required youth club development proposal. Both the application and proposal are due by the deadline stated above. It is highly recommend that you submit the application early in order to have time to develop a quality youth club league development proposal.
