As a service to its member schools, the KHSAA is offering to assist with your school's 2013-2014 T61 Title IX Interscholastic Athletics Student Survey.

The KHSAA will, at the member school's request, create an online survey using Surveymonkey that the school can in turn use to survey its students. At the end of the survey period, KHSAA IT staff will export and email the survey results to the school's principal or designee. The file will be a tabulated Microsoft Excel workbook.

Computer and internet access will have to be made available for the students in order to complete the survey.

Please complete the following survey to let the KHSAA know if your school would like to have the online survey created for them.

Since the KHSAA is both creating the survey for your school initially, and exporting the tabulated survey results to send to your school at the completion of the survey: WE ASK THAT THE END DATE IS NO LATER THAN TUESDAY, APRIL 29, 2014. Basically, this time period is the time that you will need to allow all of your survey participants to respond.

The KHSAA will email the survey results to the school's principal within 1 business day of the survey's completion date.
