Sign up to meet with TSB staff to discuss the value of certification. Use technology at the lab to apply onsite and upload required documentation. Learn about eligibility for certification and what documentation you need to bring to the session.

* 5. During the time of your application, TSB staff will walk you through the online application process. In addition to the online application, TSB certification requires additional documentation to be included with the application.

TSB staff will contact you in advance of the event to confirm your registration and to help you prepare the documents you need to bring to apply. These may be uploaded at the time of your application or at a later date.

Scanners will be available to scan your hard copy documents. If you wish to scan and upload the required documents needed for your application during your reserved time, use the list below to identify the documents required for your business type.

Documents for All Applicants

Registration of business name: County Recorder or Secretary of State

Proof of TGP status (See list below)

Two years business tax returns. If you do not have tax returns for your business, provide your personal tax returns and an IRS letter verifying the businesses' Federal Employer ID number (if using an FEIN)

Proof of liability insurance and Workers Compensation insurance, if applicable

Copy of each and every license, registration and permit required for business

Purchase agreement, if the business was acquired within the last five years

Signed and notarized application

Proof of capital contribution from each owner (if the business was started/acquired within the last five years)

Proof of TGP status for board of directors/officers. See list of acceptable proof below.

Targeted Group Person (TGP) Status Forms

If applying as a minority person or as a woman, provide documentation of your status. Acceptable documentation includes a driver’s license, passport, tribal record, birth certificate or other documentation.

If applying as a person with a disability or have a physical or mental impairment that substantially limits one or more major life activities, provide written verification from one of the following entities: Iowa Department of Education, Division of Vocational Rehabilitation, Iowa Department for the Blind, a licensed medical physician. The physician must complete and sign the Verification of Disability form which can be found HERE. This form must be signed by a licensed medical physician.

If you are applying as a Service-Connected Disabled Veteran, provide documentation from the U.S. Department of Veterans Affairs or the U.S. Department of Defense confirming your disability is service connected and the extent of the disability.
