Dear Parent,
The CDC is conducting a survey to find out about the medical conditions of public health significance seen in children from Haiti that entered the US after the January 12, 2010 earthquake. If you adopted a child from Haiti after the Jan 12th earthquake, we are asking you to please complete this brief survey. IF YOU ADOPTED MULTIPLE CHILDREN FROM HAITI, PLEASE COMPLETE THE SURVEY MULTIPLE TIMES. TO DO THIS YOU MUST LOG IN AGAIN. Your participation in this survey is completely voluntary and confidential and will take 5-10 minutes of your time. You can even change your mind after you start if you decide not to continue. Participating in this survey will not affect in any way benefits to which you are entitled to. You will not be asked to provide any personal identifiers on your children. The information you provide will help us prepare and respond to the medical needs of Haitian orphans that come to the US.
