Thank you for taking the time to complete this questionnaire about your cat's well-being. This survey will help us better understand how your cat's age and/or disease affects his or her comfort and will help us to develop better methods to deal with the issues that are associated with a cat becoming older or ill.
If you have a very young and healthy cat, you might answer every question with: "No problems at all." Although this might seem obvious, the answers from owners of young cats are very valuable to us, as we would like to find out if there is a change in a cat's quality of life when he or she gets older.
Please answer the questions for one cat only per questionnaire. Please take your time to fill in this questionnaire and try to give honest answers. If you don't know the answer to a question, or if you don't want to answer a question, you can leave it open. There is an opportunity to comment on any areas of confusion at the end of the survey.
