This is a questionnaire being conducted in conjunction with U.S. Air Force (USAF) Request For Information RFI-03-12-EGA.
The purpose of this questionnaire is to assist the USAF in assessing the ability of roofing replacement and repair contractors to successfully operate under new Department of Defense (DoD) requirements and several potential procurement scenarios. These scenarios are divided into Regional and National. Questions in each scenario may be similar, but are necessary to understand any differences in the ability of roofing contractors to provide service given different operating parameters. These scenarios are provided for information purposes and does not imply a government obligation or commitment to implement.
The questionnaire consists of five (5) pages in total and may require up to 30 minutes to complete. Questions with an asterisk (*) are required. Please complete this survey by 15 June 2012. To advance the survey, click the "Next" button. To go back a page, click the "Prev" button. Do not use your internet browser icons to navigate the survey. If necessary, you may exit the questionnaire while in process and resume on the same page at a later time. Please ensure you use the same computer and have cookies enabled.
