Informed Consent
The following study is designed to assess college students' perceptions of creativity and beliefs about sexuality. This study is being conducted by Dr. James C. Kaufman, Professor of Psychology at the California State University, San Bernardino (CSUSB). This study has been approved by the Psychology Department Institutional Review Board subcommittee of the California State University, San Bernardino. A copy of the official Psychology IRB Committee stamp of approval should appear somewhere on this consent form.
In this study you will be asked to complete a survey asking for your self-perceptions about your creativity and your beliefs about sexuality. The online survey should take approximately 70 minutes to complete. All of your responses will be completely anonymous; your name will not be collected and the researcher will not share the data outside of the research project. If you are a CSUSB student, you will be asked to provide your name and SONA ID for extra credit points. Contact information for SONA extra credit will be stored separately from the survey responses in order to protect the anonymity of your responses. All data will be reported in group form only and stored in a CSUSB password protected computer. Data will be destroyed seven years after publication. Summary results of this study will be available no earlier than September 30, 2013.
Your participation in this study is entirely voluntary. You are free to withdraw your participation at any time during the study without penalty, and to not answer any questions that make you uncomfortable. This study entails no risks beyond those routinely encountered in daily life, nor does it provide any direct benefits to individual participants. However, you may learn more about creativity and/or yourself beliefs from participating in this study. If you are a CSUSB student, at your instructor's discretion, you may receive 3 units of extra credit for participating in this study.
