2011 National Health Center Capital Needs Assessment |
Instructions
Thank you for participating in this health center capital needs assessment. Results will be used to estimate the need for additional capital investment to allow health centers to serve up to 40 million patients and to inform strategies for securing capital funding. The assessment has four sections:
Section 1 covers your health center's contact information and current operations.
Section 2 should be answered if you have one or more specific capital projects, which may be in the early planning stages or much further along. Please do your best to answer the questions based on what you currently know about your projects. For instance, if you have not secured site control but know the town where you want to locate your site, please list the city, state and zip in the spaces provided. You will have the opportunity to provide information on up to five capital projects. At the end of each project simply indicate if you have any additional projects to add. If you do not have any specific capital projects you will automatically be skipped forward to Section 3.
Section 3 covers your health center's existing capital infrastructure.
Section 4 covers your health center's capital planning needs and obstacles.
