Voice of the Sea Survey Consent Form
Aloha! My name is Kanesa Duncan Seraphin. I am a faculty member at the University of Hawai‘i Sea Grant Center for Marine Science Education. The purpose of this survey is to assess people’s perceptions about the television series Voice of the Sea.

What am I being asked to do?
Fill out a survey based on the 25-minute episode of Voice of the Sea.

Taking part in this study is your choice.
Your participation is completely voluntary. You may stop participating at any time. If you stop being in the study, there will be no penalty or loss to you.

Why is this study being done?
The purpose of this project is to assess people’s perceptions after viewing Voice of the Sea. The findings may help to better understand the educational experiences of those who watch the television series Voice of the Sea. I am asking you to participate because you recently viewed an episode.

What will happen if I decide to take part in this study?
The survey will consist of 15 statements that you will be asked to rate on a 1-5 scale, 2 multiple choice questions, and 1 open-ended response opportunity. It will take about 5-10 minutes. The survey will include statements like, “Before and After viewing this episode of Voice of the Sea, please rate your understanding of the importance of conserving fresh water. ”

What are the risks and benefits of taking part in this study?
I believe there is little risk to you for participating in this research project. If you do become stressed or uncomfortable answering any of the survey questions, you can skip the question or take a break. You can also stop taking the survey or you can withdraw from the project altogether.

There will be no direct benefit to you for participating in this survey. The results of this project may help improve the Voice of the Sea series to benefit future viewers.

Confidentiality and Privacy:
You will have the option to join our Voice of the Sea email list by providing your name and email. Your name and email will not be shared. I will keep all study data secure in a locked filing cabinet in a locked office/encrypted on a password protected computer. Other agencies that have legal permission have the right to review research records. The University of Hawai'i Human Studies Program has the right to review research records for this study.

Compensation:
There is no compensation for participating in this research project.

Future Research Studies: 
Identifiers will be removed from your identifiable private information and after removal of identifiers, the data may be used for future research studies or distributed to another investigator for future research studies, and we will not seek further approval from you for these future studies.
