By requesting and accepting PPE recipient agrees and understands the following:

• The Recipient is receiving Personal Protective Equipment (PPE) from the Ohio Emergency Management Agency/Ohio Department of Health which was purchased with federal CARES Act funds.

• PPE may include some or all of the following items: gloves, masks, gowns, etc., as requested by the recipient.

• Upon receipt of the PPE it shall be the sole responsibility of Recipient to utilize the PPE consistent with any applicable local, state, and federal laws

• It is understood that the type of PPE being provided are NOT medical grade, and the State of Ohio makes no warranty, or representation, expressed or implied, regarding the design or condition or fitness for any particular purpose.

· PPE should be used in accordance with OSHA regulations. Workers must be trained on and demonstrate an understanding of when to use PPE, and how to appropriately don and doff PPE.

· PPE should be stored in a clean dry place, not near direct sunlight or heat

• The PPE may not be returned.
