Welcome to HPCI’s (Herts Parent Carer Involvement) third annual survey of Parent Carer Satisfaction

This survey is for parents and carers in Hertfordshire who have children and / or young people aged 0 – 25 years, with SEND (Special Educational Needs and/or Disabilities). Your responses will drive our work with Hertfordshire County Council and Hertfordshire's NHS Services to help improve services for children and young people with SEND.

We run this survey every year so we can see what's improving and what issues families are still facing. Because of this it is really important that you only tell us about your experiences during the last school / college year – from September 2023 to August 2024. We will remind you in questions by calling this "the last school year".

Please complete the survey thinking of ONE child or young person only. If you have more than one child with additional needs, please complete a survey for each of them if you can.

Depending on the needs of your child, our survey could take up to 30 minutes to complete, and if you would like to share additional comments with us, we have left spaces for you to do this.

The survey will close on Sunday 3rd November 2024 at 11pm.

Please note:

  • Wherever we have used the term “child” in the survey we mean your child or young person.
  • Wherever we have used the term "NHS Therapies" we mean services such as Occupational Therapy (OT), Speech and Language Therapy (SaLT), Physio Therapy etc.
  • The Special Advisory Service teachers (e.g. teachers for the Deaf or teachers for Visually Impaired) are not NHS therapies.
  • If you want to know more about NHS services, you can read a quick introduction here.
  • Sometimes we ask the same or a very similar question again - this is so the software we use can filter you to the next set of relevant questions.
  • We know that not every question will have the perfect answer for you, please answer as best you can.
  • There are a couple of security questions in the survey to try to eliminate bots. Please leave question 8 blank.

Your Privacy
We may work with an independent third party to help us analyse the data, they will keep all data confidential. We will never share any information that might identify you or your child. We may share our summarised findings from this survey with the people who are responsible for providing services to disabled children in Hertfordshire. Our aim in doing this will be to try and help improve these services in the future. We will remove any information that may enable someone to identify you, or your family, before sharing any data outside HPCI. If you would like to read our privacy policy, it is available here.
