Introduction and context for this survey

The Ontario Intensive Treatment Pathway (OITP) is working to build a better future for intensive child and youth mental health treatment in Ontario, beginning with live-in treatment.

This survey is for family members/caregivers in Ontario who have supported a child/youth through intensive child and youth mental health treatment (for example live-in treatment or day treatment), and is designed to help us better understand the financial impacts and personal costs to families and caregivers.

The OITP is committed to improving supports for families and caregivers in the future. Recognizing we will not be able to address all of the financial impacts, personal costs and barriers at once, the findings of this survey will help the OITP prioritize where we could begin making improvements that will make the biggest difference for families and caregivers.

Please know that completing this survey is voluntary and your responses are anonymous. We will not ask your name and results will not be tied to any specific respondent. Aggregated results will be used to inform learning and improvements to child and youth mental health. We expect this survey to take 10-15 minutes to complete, and you can return to the survey to finish completing it at a later time if you need to.

Completing the survey may bring up difficult experiences for yourself and your family – please prioritize your own well-being, take your time and step away if needed. You can also find more information about supports available to families and caregivers, including caregiver peer support, at Parents and Caregivers for Mental Health. More options for support are listed at the end of the survey, and you can skip ahead to the "Thank You and Next Steps" page at any time to find this list of resources.
