As we prepare to offer the COVID Vaccine to our community, it is important for us to obtain an understanding of how many people within our community are interested in being vaccinated and of what age groups. We would like your name, email and phone number in order to contact you with updates or as needed during vaccination times, but this is not required at this moment. Feel free to pass this survey along to others that live within our community or help others that don't have a computer. Also, please understand that we may get either the Pfizer or Moderna Vaccine in our community depending on availability.
