The Cumming-Forsyth County Chamber of Commerce, in an effort to support entrepreneurship in our
community, is excited to host the 2016 Excellence In Business Awards Gala. This program offers well deserved recognition to small businesses for their achievements and contributions to our community by providing the opportunity to showcase their company, share their story and be rewarded for their efforts.

APPLICANT BUSINESSES MUST MEET THE FOLLOWING CRITERIA:

-Be a member of the Cumming-Forsyth County Chamber of Commerce
-Have been operational for a minimum of one year
-Be the primary owner or major shareholder of the business
-Demonstrate an increase in sales and /or unit volume over a three year period
-If applicant is a new business, indicate the unit of sales
-Previous winners are ineligible for two years
-Members of the Advisory Council are not eligible

It is the applicant’s responsibility to verify the above criteria. The Cumming-Forsyth County Chamber of
Commerce is NOT responsible if applicant is disqualified for not meeting criteria.
Applications are due to the Cumming-Forsyth County Chamber by Friday, March 18, 2016.

The winner will be announced during the 2016 Excellence In Business Awards Gala on Thursday, April
21, 2016.
