The Evanston Health & Human Services Department and Illinois Department of Public Health are adopting CDC guidance allowing a second COVID-19 booster dose for individuals 50 years and older and immunocompromised individuals 12 years and older.
  • Individuals age 50 and older can get a second booster dose of either the Pfizer or Moderna vaccine if it's been at least four month since their first booster dose.
  • Immunocompromised individuals age 12 and older can get a second booster dose of the Pfizer (12+) or Moderna (18+) vaccine if it's been at least four months since their first booster dose.
  • Individuals ages 18-49 who are not immunocompromised and who received the J&J vaccine as their primary and booster dose may receive a second booster dose of the Pfizer of Moderna vaccine if it's been at least four months since their first J&J booster dose.
It's recommended to schedule an appointment by visiting vaccines.gov. You can select a time and location that is most convenient to you.

However, if you would like to receive a booster dose from the City of Evanston's Health & Human Services Department, please fill out this short survey. By completing this form you are not scheduling an appointment but will be added to a list to receive updates on the booster shot process.

This survey is HIPAA compliant, meaning your health information will be protected.
