In-School/Distance Education Mandatory Questionnaire

For 2020-2021 JICS Parents and Guardians

We understand that for some parents, the risks associated with in-school learning are too significant for their family due to a medical situation or family circumstances that make returning to school imprudent.  For those children we will offer remote learning accommodations.  In order to plan for distance education, we ask that parents complete this short survey that will inform us of the children who will require distance-education so that we can better prepare for September. 

Please complete the survey by Wednesday, August 19, 4:00pm.
If you have more than one child attending JICS, please fill out a separate survey for each child.
