Your opinion matters: Dominion National is conducting a customer satisfaction survey regarding the use of your dental benefits. In this survey you can also submit your information to receive reimbursement for your $10 office visit copayments. Simply complete the requested information at the end of the survey and Dominion will send you a check upon confirmation of your cleanings.
You will need the following details to submit for Prevention Rewards:
• Member ID # (located on your dental ID card)
• Subscriber and dependent(s) date of birth
• Date of cleanings
The submission deadline is 4/1/22. Payments will be issued by 4/30/22.
